I am currently looking to recruit a band 6 social worker or community mental health nurse to join the community intervention mental health service in East Ham.

The role is Monday Friday, 09:00am 17:00pm, 37.5 hours per week.

Pay: Rate will depend on expereince, the only payment option is through an umbrella company and will be inside IR35. 

Job Summary: The post holder will undertake assessments and short-term clinical interventions with patients in primary care in order to support PCNs in assessing and treating patients within a biopsycho-social framework.

The post holder will make use of referral data and information available through the GP contract (Quality & Outcomes Framework) in conjunction with their clinical lead, as a basis for discussions regarding Mental Health. They will work to increase awareness of mental health issues within Primary Care.

The post holder will link closely with the Primary Care clinics and the catchment area secondary care MH teams, attending referrals meetings to expedite primary care referrals and provide feedback as required.
