Job Title: Qualified Social Worker (QSW) - Adulthood Services Team

Location: Shropshire Council, Shirehall

Rate: 32 per hour

Position Overview

As a member of the Preparing for Adulthood adult services team, you will play a vital role in supporting the delivery of Shropshire Councils transformation program. Working in collaboration with a multidisciplinary team, including Social Workers, Community Mental Health Nurses, and Occupational Therapists, you will contribute to building a healthy and inclusive society in Shropshire.

Key Responsibilities

Undertake assessments and care act responsibilities for young people aged 16-25 within a rural geographical area of Shropshire.

Contribute to the Multi-Agency Protection of Vulnerable Adults procedures and investigation processes.

Work in partnership with health, housing, and the private and independent sectors to promote a holistic approach to social care.

Level And Purpose Of Post

You will work under the guidance of the Team Manager and Senior Practitioner, focusing on the well-being of vulnerable adults, including individuals with disabilities and their caregivers.

Main Duties

Provide a social work service to vulnerable adults, adhering to the relevant Acts of Parliament, Council policy, and Partnership policies.

Collaborate with colleagues in the MDT to ensure comprehensive and integrated care for service users.

Work within the framework of departmental accountability to address the needs of individuals and their families.

Requirements

Qualified Social Worker with relevant certifications.

Experience In Assessments And Care Act Responsibilities.

Familiarity with the Multi-Agency Protection of Vulnerable Adults procedures.

Ability to work in a rural geographical area of Shropshire.

Excellent communication and collaboration skills.
