Location: Oxfordshire County Council
Rate: 35 per hour

Job Overview
The successful candidate will be responsible for community-based assessment and equipment provision, playing a vital role in improving the quality of life for our clients. The use of Liquid Logic is beneficial, but not essential, as we provide comprehensive training to support your professional development.
Key Responsibilities
Conduct thorough assessments using community-based approaches to identify clients needs.
Formulate and implement personalized treatment plans, including the provision of necessary equipment to enhance clients functional abilities.
Collaborate closely with interdisciplinary teams for effective care coordination.
Utilize Liquid Logic for documentation, and training will be provided for those unfamiliar with the system.
Conduct remote assessments when feasible, demonstrating flexibility in the delivery of care.
Embrace a flexible work schedule, with some home-based working and the possibility of desk work outside ordinary hours.
No home visits outside of Monday to Friday, 9:00 AM to 5:00 PM, ensuring a healthy work-life balance.
