Randstad are supporting one of our longest standing NHS clients based in the North East who are looking for an ambitious and experienced nurse to take the lead as a case manager in their CHC service, within the mental health and learning disability team.

This is an exciting opportunity for career development and a chance to make a difference and improve the day to day lives of those individuals with highly complex health and social care needs.

This is a full time role working 37.5 hours per week, Monday - Friday.

The successful candidate can expect attractive pay rates of 300+ per day, this role is a mixture of remote/office/community based working so living local to Newcastle upon Tyne is ideal but not essential.

The responsibilities of the CHC Nurse Assessor will include:

  • Continuing responsibility for a caseload of patients.
  • To ensure identified priorities are addressed and improvement outcomes are

demonstrated for Care Providers

  • To undertake holistic assessments in conjunction with the Multidisciplinary Team for

patients identified as meeting CHC. This will include liaising with other professionals,

carers, and family members to identify care needs and plan to meet these needs.

  • To identify the risks presented by the needs of individuals and develop person centred,

cost effective care plans, working with specialist health professionals to manage these

risks and individuals needs.

  • To guarantee providers are evidencing that package of care are based on patient needs

to ensure value for money.

  • To monitor and review care packages with other professionals involved in the care

provision, undertaking scheduled and unscheduled reviews for patients with CHC needs,

ensuring that the care provision, quality of care and the setting is appropriate to need.

  • Develop and evaluate delivery of care through continuous review of evidence based

clinical standards and ensure these are within agreed policies and protocols

  • Support and facilitate ongoing professional development.



The successful candidate will be able to demonstrate:

  • Strong understanding of the CHC National Framework
  • Highly motivated and able to work autonomously
  • Excellent written and verbal communication
  • Excellent people management skills with the confidence to work with individuals with varying clinical conditions

The minimum requirements for this role are:

- NMC registered

- Nursing qualification (Mental Health/General/LD)

- Experience as a case manager with continuing healthcare experience
