38/hr - Hospital Care Management - Social worker (North West England)

Contract - Full time

Looking to earn a generous hourly rate of 38/hr , whilst having the opportunity to work remotely 4 days a week, allowing you to tackle caseloads from the comfort of your own home.

About the role:

• Dealing with caseloads revolving around adults undergoing discharge planning.
• You'll be working closely with senior practitioners, and reporting to the team manager.

What you'll bring to the table:

• At least 1 years post-qualifying experience as a social worker
• Considerable learning disabilities experience.
• Hard work and commitment, a strong and convincing display of a drive to succeed.
• Solid communication skills and relationship building capabilities.
• DBS Checked
