Tripod are seeking a dedicated and compassionate Social Worker to join our Through Care team in Rutland. As a Social Worker, you will play a crucial role in providing support and guidance to individuals and families navigating the challenges of through care. This position offers an opportunity to make a meaningful impact on the lives of those in need while working within a supportive and collaborative environment.



Key Responsibilities:





  1. Assessment and Planning:




    • Conduct thorough assessments of individuals and families to identify their unique needs, strengths, and challenges.

    • Develop comprehensive care plans in collaboration with clients, incorporating their input and preferences.




  2. Case Management:




    • Provide ongoing case management and support to individuals in through care, ensuring the effective implementation of care plans.

    • Advocate for clients' rights and access to appropriate services and resources.




  3. Crisis Intervention:




    • Respond promptly and effectively to crises, providing emotional support and crisis intervention as needed.

    • Collaborate with relevant agencies and professionals to address urgent situations.




  4. Collaboration and Networking:




    • Work collaboratively with other social workers, healthcare professionals, and community resources to coordinate holistic care for clients.

    • Establish and maintain strong partnerships with local organizations and agencies to enhance support options.




  5. Documentation and Reporting:




    • Maintain accurate and up-to-date case records, ensuring compliance with relevant regulations and standards.

    • Prepare detailed reports for internal and external stakeholders, summarizing client progress and outcomes.




  6. Client Empowerment:




    • Empower clients to actively participate in decision-making processes regarding their care.

    • Provide information, resources, and skills training to enhance clients' independence and self-efficacy.





Qualifications and Requirements:




  • Bachelor's or Master's degree in Social Work or a related field.
