We are currently working with a Local NHS Trust who are seeking a Band 6 Community Psychiatric Nurse to join their Dementia Service in Hounslow.

What s on offer:

• Working hours: Monday-Friday, 9am-5pm
• Contract length: 3-6 month initial contract with the view of extension
• Location: Hounslow
• Rate of pay:  28p/h Umbrella (negotiable depending on experience)

About the role:

• Undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk.
• Work alongside the service user to develop their care plan along the recovery principles.
• Assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.

Requirements:

• Registered Mental Health Nurse with a valid NMC Pin
• Previous experience working with dementia patients in the community
