Band 5 Community Nurse - NHS

A local NHS Trust we are currently working with are seeking a Band 5 Community Nurse to join their team in Hampstead.

Team: Integrated Community Healthcare Team

Pay rate:  22 p/h (potentially negotiable depending on experience)

Contract: 3-month initial contract with potential for extension

Working Hours: Monday - Friday, 09:00-17:00

Location: Community based in Hampstead

Key responsibilities will be:

• Give skilled nursing care, based on principles of best practice and clinical evidence. This includes: Chronic disease management, end of life care, continence promotion and wound management.
• Take continued responsibility for the holistic health needs assessment including care planning, evaluation and appropriate implementation of health care and ensuring continuity of care following referral from acute care, rehabilitation units, GPs and other agencies.
• Undertake comprehensive assessment of the physical, psychological and social care needs of patients using the Single Assessment Process and including the needs of carers.
• Provide individualised and person centred care plans that provide monitoring and promotion of self care to prevent unnecessary hospital admissions

To be eligible for these positions, you will need to be a registered Nurse with a valid NMC pin and have experience working in the community.
