• Undertake care coordination and care management.
• Assesses needs and identifies problems relevant to the care of people referred to.
• Devises a plan of care in partnership with the person and carers.
• Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multidisciplinary team as a member of a CMHTOP service.
• Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
• Implements the planned programme of care to ensure a high standard is achieved.
• Advises and supports the person and carers to promote health and well-being and to prevent illness.

What you'll need to succeed

• Up-to-date Professional Registration (to be maintained), RMN
• Evidence of continuing professional development in the field of mental health
• Experience with older adults with mental health problems and lone working
• Ability to apply therapeutic knowledge to clinical practice
• Experience in Mental health and risk assessments
• Community Experience
