The pay rate for this exciting role is up to  30 an hour and is based in Blackburn.

Key Responsibilities:

• Work with adults in a community setting who are at risk of hospital admission.
• Conduct urgent assessments and provide appropriate interventions and referrals.
• Ensure compliance with relevant policies, procedures, and legislation.
• Monitor and review the progress and outcomes of your clients.
• Participate in regular supervision, team meetings, and training sessions. 

Essential Requirements:

• SWE registration
• Full UK drivers license 
• Clear, enhanced DBS (We can provide this if you need it)
